Register with the surgery

Find your local GP Practice and register as a new or temporary patient.

You need to speak with one of our receptionists prior to registering with the practice, to ensure the necessary documentation. 

There are 2 registration forms for different circumstances in NI

  • Patients wishing to transfer GP within Northern Ireland or transferring from Great Britain need to complete a form HS200.
  • Eligible visitors or persons taking up ordinary residence in Northern Ireland need to complete a form HSCR-1.

You are also required to fill out our practice registration document and provide photographic ID and proof of address within our catchment area. 

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to request to see any GP or nurse in the practice, we endeavour to faciliate this where possible.
